In most instances your service provider will be able to bill Canada Life directly. However, in some cases you will be required to pay for the service yourself, and then submit a claim form for reimbursement.

To obtain reimbursement for services, pay for the service and obtain an official paid-in-full receipt.
Download and print the appropriate claim form (below), complete it in full, and then submit it along with the receipt to Canada Life for processing.
